Overview

Breast augmentation uses breast implants or fat transfer to increase breast size, restore volume lost after pregnancy or weight changes, and improve breast shape and symmetry. The procedure can increase fullness and projection, improve breast-to-hip balance, and address congenital or acquired breast asymmetry.

Implant options include saline-filled, silicone gel-filled, structured saline, and form-stable (gummy bear) varieties. Implants come in round or anatomical shapes and smooth or textured surfaces. The choice of implant type, size, shape, and placement is individualized based on the patient's anatomy and goals.

Breast augmentation does not correct significant breast sagging. Patients with drooping breasts may require a breast lift (mastopexy) in addition to or instead of augmentation to achieve their desired result.

What to expect

  1. Initial consultation to discuss goals, review medical history, and develop a personalized surgical plan
  2. Pre-operative evaluation including breast measurements, photographs, and imaging as needed
  3. Selection of implant type (saline or silicone), size, shape, surface texture, and profile
  4. Determination of incision location (inframammary, periareolar, or transaxillary)
  5. Administration of anesthesia (general anesthesia or intravenous sedation)
  6. Incision creation at the predetermined site
  7. Development of the implant pocket (submuscular or subglandular placement)
  8. Insertion and positioning of the breast implant
  9. Layered closure of the incision with sutures, skin adhesive, or surgical tape
  10. Application of dressings and a support garment or surgical bra
  11. Post-operative monitoring and discharge with detailed recovery instructions

How a breast augmentation works

  • The surgeon makes an incision in one of several locations: along the inframammary fold (under the breast), periareolar (around the edge of the areola), or transaxillary (in the armpit). Incision choice depends on implant type, desired enlargement, patient anatomy, and surgeon preference.
  • A pocket is created either behind the breast tissue and in front of the chest muscle (subglandular placement) or behind the pectoral muscle (submuscular placement). Placement depends on implant type, body type, and the degree of enlargement.
  • The selected implant is inserted into the pocket and positioned for optimal symmetry and contour. Incisions are closed with layered sutures in the breast tissue and with sutures, skin adhesive, or surgical tape on the skin surface.

When it's recommended

  • Desire for increased breast size and fullness
  • Breast volume loss following pregnancy, breastfeeding, or weight loss
  • Breast asymmetry where one breast is noticeably different in size or shape
  • Congenital breast underdevelopment (breast hypoplasia)
  • Tuberous breast deformity correction
  • Breast reconstruction following mastectomy or injury
  • Dissatisfaction with breast shape or projection

Recovery & aftercare

  • Initial recovery typically involves several days of reduced activity and rest
  • A support garment or surgical bra is generally worn for several weeks
  • Swelling, bruising, and tenderness are common in the first 1-2 weeks
  • Most patients can return to non-strenuous work within 1-2 weeks
  • Strenuous exercise and heavy lifting are typically restricted for 4-6 weeks
  • Final results may take 3-6 months as implants settle into position and swelling fully resolves
  • Regular follow-up appointments are needed to monitor healing and implant integrity

Alternatives

  • Fat transfer breast augmentation (autologous fat grafting for modest size increase)
  • Breast lift (mastopexy) for improved shape without size increase
  • Combined augmentation-mastopexy for patients with both volume loss and sagging
  • External breast prostheses (non-surgical option)

Frequently Asked Questions

  • Breast augmentation is a surgical procedure that uses implants or fat transfer to increase breast size and improve shape. It can restore volume lost after pregnancy or weight changes and address breast asymmetry.
  • Breast augmentation is performed under anesthesia, so patients do not feel pain during the procedure. Post-operative discomfort is common for the first few days and is typically managed with prescribed pain medication. Most patients report that discomfort diminishes significantly within the first week.
  • Breast augmentation using FDA-approved implants has an extensive safety record, and the vast majority of patients experience no serious complications. As with any surgery, there are risks that are discussed in detail during the consultation process.
  • Breast implants are not considered lifetime devices. The average lifespan is approximately 10 years, though many implants last longer. Regular follow-up with a surgeon and imaging as recommended by the FDA can help monitor implant integrity over time.
  • The main types are saline-filled and silicone gel-filled implants. Saline implants are FDA-approved for patients 18 and older, while silicone implants are approved for those 22 and older. Both come in various sizes, shapes, and profiles to suit individual anatomy and goals.
  • Breast augmentation may not be appropriate for individuals with active breast infections, untreated breast cancer, those who are pregnant or breastfeeding, or patients who have not reached physical maturity. A thorough consultation helps determine candidacy.

Breast Augmentation risks & candidacy

Who should avoid this

  • Active infection or untreated breast disease
  • Existing breast cancer or pre-cancer that has not been adequately treated
  • Pregnancy or currently breastfeeding
  • Patients who have not reached physical maturity (under 18 for saline, under 22 for silicone per FDA guidelines)
  • Autoimmune conditions that may be exacerbated by implant placement (requires careful evaluation)
  • Unrealistic expectations regarding surgical outcomes

Possible risks

  • Capsular contracture (formation of tight scar tissue around the implant that may distort breast shape)
  • Implant rupture or leakage requiring additional surgery
  • Changes in nipple or breast sensation (may be temporary or permanent)
  • Infection requiring treatment or implant removal
  • Hematoma (collection of blood) or seroma (collection of fluid)
  • Implant malposition or asymmetry
  • Wrinkling or rippling visible through the skin
  • Poor scarring or hypertrophic scar formation
  • Need for reoperation or implant replacement
  • Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL), a rare cancer of the immune system
  • Breast implant illness (BII), reported systemic symptoms including fatigue, joint pain, and cognitive difficulties
